Still working for me, you might be getting the switching thing wrong, here's some more examples.
Make sure you're swapping by two's and not one at a time.

02.14 > 14.02
09.45 > 45.09
85.76 > 76.85
AE.41 > 41.AE

The blue periods are only to see the change easier.

I've read this a few times over and still can't find which numbers to switch and how exactly to switch them using Hex editor.

Now, if you look on the left, you will see a space with '0' in it, and the

n 2 boxes with 2 sets of numbers.

^^ been trying to figure out what that means with the picture.

What I'm trying to do is replace my King's Defender sword with a Thunderfury. I found both numbers. Just need to know which numbers to switch and how.
