I've seen people fly from Netherstorm to Ghostlands... but I am not sure that I have seen the flight from Exodar to Hellfire Peninsula.
Thanks much for bringing this exploration to the forefront again... it is always good to remember these locations are all on the same map, and we're always up for a challenge!
Would you not either encounter invis walls, some type of fatigue/dismount/killwall, or most obvious disconnect from being too far out..........?

This has been done multiple times without hacks. I've done the Silvermoon flight before, nothing too fancy. Flying in Silvermoon is pretty cool but there are other methods for that. Anyways,
All that's needed to pull off a flight to Exodar is a 'Flying without mount' exploit, but afaik there are no public ones out there. Could probly be reached using Control FP aswell.
There's no mechanic disconnecting you from being to far out. No kill walls exist.
There's a bunch of walls out there, but if you're using one of the two exploits I mentioned then walls won't be causing you that much trouble. Fatigue zones exist but that's no issue. When you've passed the first 2-3 walls you're fine.

The most recent one involving becoming a panda was patched but a method still exists for druids to fly without a mount, although very slowly. Druids can Displacer Beast through invisible walls just like Mages can blink. There is another method for any class but you will have to wait until Pilgrim's Bounty.
Here is a reference map of the Outlands instance. You can use it to determine which direction to head in.
It will be interesting to see if the developers copy+paste this zone to make Draenor in WoD. If so there may be instances of Eversong Woods and Azuremyst Isle left inside the Draenor zone.

Again, I've seen (and done) Netherstorm to Ghostlands/SIlvermoon, but never actually seen Hellfire to Exodar done. Now that I see the map you posted, I'd be curious to see someone do Exodar to Ghostlands!
Back in Cataclysm (?), I made the flight from Shadowmoon to the Exodar using the broom mount, mushroom chair, and sandbox tiger. Going from Exodar to Ghostlands is feasible, but you'd have to get to Exodar with flight and make it past the fatigue zone (three times, I'd imagine).
Actually, I may have just thought of a way to get from one to the other. Will need to look into it and ask around. Though, with what I'm thinking, I imagine it would have been done before.
I like challenges! Soon as I get back on my WoW feet I'm going to give this a go!

Here is a video I put together from some old footage I had. This exploit has since been fixed. Sorry for the fast camera movement I didn't want too many cuts in the video but I also didn't want a 22 minute video :P
