I think i know whats causing this, I am a Australian and our server time is diffent to the instance times, So its 5pm on wednesday here but its 10pm tuesday in the instances and because you can run the same heroics and it ignores lock outs I believe its just "Forgetting" that you had done that because you are changing Server time zones.

It is true, it may have stopped now though, due to the times?

Edit: It may have been fixed, it now says I'm getting no reward at all for doing more than one dungeon? Anyone else?

Yes i'm on the bloodlust battlegroup.
I'm getting rewarded 2x frost everytime I run a instance, while the quest to start it off says triumph, I get frost at the end.
Up to 19 now haha
