I think if you print out the form, and then scan it, they will accept it, as thats what I did, but they found out I was selling.
Was your simply an ID needed or did you need a notary?

1st page says it has to be mailed. You can try faxing but i doubt it will work. Blizzard is very strict with their rules.
Account-Retrieval.pdf, the normal lock, can be faxed, mailed, or emailed.
Account-Retrieval-Notary.pdf, the notary lock, can only be mailed.

I live in the UK and unlocked my US account when using UK stamps etc so it works in whatever country your in as far as im aware.

Just to make Blizzard's 3 forms of locks clear:
Minor Lock:
They ask for 3 pieces of info: account name, first name, and last name.
By "acknowledgement that account has been secured," they pretty much mean your computer. They want you to scan it for viruses and spyware. Other than that though, as long as you know the first and last name, you're good.Originally Posted by Blizzard Minor Lock

Requires photocopy of ID and a form. The form asks for the first name, last name, and other info. The address, city, state, ZIP code, etc. don't really have to match on the account, but the first and last name definitely do. This can be mailed, faxed, or emailed. Faxing can make the process difficult. What you see on your end can be very different from what Blizzard receives. If it's too small, too dark, too light, they can't accept it. Fax line noise and other technical problems usually cause that. Considering you can email this form, there's really no reason not to email it.
Notary Lock:

Requires the same info as the normal lock, except the ID and form have to be notarized. Notary publics notarize documents and such. In the US, you can find them at banks, legal institutions, and some other places. Anyway, they'll basically just put their little notary seal on it and fill out a form saying that you are who you say you are. This can only be mailed.
The easiest place to find a notary public is at a bank, so, I'd highly recommend just going to your nearest bank.
Hopefully this clears things up for some people who are confused about locks.
