Are mildly inappropriate references to human anatomy or bodily functions.
Are references to illegal drugs or activities.
Have racial, ethnic, or national connotations.
Have any connotations of major religions or religious figures (e.g. Jesus, Christianity, Buddha).
Include names of World of Warcraft realms, zones, or names of major characters from Warcraft lore.

In the past when this happened, the non-offensive names stayed. No one was banned, and people still play those characters.
I can't really see why they'd ban for simply creating a character with no special process or 3rd party program, unless of course you're going out of your way to make names as offensive as possible. At worst I foresee characters being removed or force deleted.
I had the name Aids. That shit writes itself. Had a forced name change by the time he hit level 5.

Hmm, what about some advertising names? Were these allowed? Like mygoddotcom, mygoldcom etc?
Also I think it is important to play a bit with your character a bit, it will make harder to remove it imo. Bliz really value players' time and this might help to save your unusual names (for example when diablo 3 rmah overflow gold dupe happened, bliz didn't do a rollback but tried to remove all the duped gold manually). Maybe the trick is to take a very unusual name that does not violate any rules (bosses/realm/mobs names does).
