I had the same idea (/tar Millhouse) but can confirm that a targeting macro does not work for Horde -the distance from Horde architect table to the Inn is too far. If are Horde you must leave the Town Hall and start spamming the targeting macro until you are in range, I made two macros, one for an NPC in the Inn and the second for Millhouse -that way I could confirm that I was in range and that there was no Millhouse.
Alas, I haven't been able to replicate OP... but I will continue to try,

Did you have the Weekly 'Follower recruitment' completed or not? (Updated) Recruiter NPC in Tavern and uncompleted.
EDIT: Finally got him! Yes I can confirm that in my Garrison (lvl 3), that I had to run outside Town Hall and run target macro -it did not detect Millhouse from the Horde Architect Table! It took about 10 to 20 swaps, running outside each time but roughly 10 minutes to get it. Once completed (tank, instant queues for the win), Millhouse was gone so I swapped the Tavern and Lumber Mill again to respawn him -this only took one swap.

I am not too sure about this. When I told a guild mate to do this he would do a double switch than /target and it worked. So basically what he was doing is switching it from the close side to the far side, waiting 2 seconds, than switching it back to the close side (meaning closer side to architect table) and waiting 2-3 seconds than hitting his macro with /target millhouse and it worked. This way he did not have to run outside each time to look for a quest marker on his mini-map. So this is definitely working on horde with a /target as long as the building is on the close side.
To answer your other questions.
1) I think your asking were there other quests in there with millhouse when he spawned? If so the answer is yes.
2) I honestly do not know but I don't think this has an affect. As you can tell by how big this thread is it works under pretty much all circumstances that I know of. Just keep at it and understand it can take a ****-ton of tries according to some people.

RE: Target macro. The target macro Horde (Garrison lvl 3, Tavern 2, War Mill 1) not working for me even when Tavern was moved closest to Town Hall. If I did not go outside (closer side) then I would not have found Millhouse. You can verify this yourself by making a targeting macro for an NPC in your Tavern and see if it pops from Architect Table.
It took me > 20 but less than 50 attempts -a lot of those were possibly wasted because I wasn't going out and running the macro in the beginning. Once I was running outside it took less then 10~20 swaps and roughly 10 minutes in all.

Used nazgul's method and it took me about 15 switches which was like 3 mins+7 rep to both of you!
Worked, as horde i just stood at the table and switched between the two plots and hit the target macro everytime the building was close to me. He showed up after 20 minutes, save on all the running in and out. +rep
Just did this and it worked. Note that as an alliance I had to run out of my Town Hall for the /target macro to target him for some reason
