This should do it.
Rename it to patch-x.MPQ.
And run a model edit fix.
This isn't a WDB change, it's DBC.
To answer your question...yes, for WDB swaps you need a model edit fix and you have to replace the old one. If you want to get rid of an edit, just delete the itemcache.wdb and login to WoW, you'll receive a new one.

1) He didn't use your itemcache to make the model swap.
2) The Bloodskull destroyer can't be found inside your itemcache.wdb (which you posted above).

Make a decision, by saying only two races without the genders, you don't do anything.
