That's either Bartender or Bongos, and the CyCircled mod to make the buttons look that way. There's a handful of action bar mods, but those are the most likely.

Couple things i would change, shorten party frames and have players target moved across so it sits on top of player frame, also move party frame to top of screen, i like to see more of whats happening. Never been keen on btm part of screen took up by chat frames etc, other than that its fine. Whats the raid mod you use?
