- Name: Your full name, which the account was created with.
- Address: The full address given when the account was created.
- Secret Question and Answer
- Last 4 digits on the visa/credit/debit or pre-paid game card that you used
IMPORTANT: For security reasons please include all the above information in a text file (e.g. Notepad), as an attachment to your reply.

- A scanned copy of a valid photo ID (passport, national ID) taken on most recent, local newspaper **needs to have the expiry date and your picture on it, please do not crop the pictures.
