you dont need to have an item in the slot at all times. I did it with a trinket I bought by mistake. But also there is only a few items you can buy from emblem of valor. Not a full set

No you don't just as the same reason if you have one Ilvl 213 ring or trinket you could switch slots on them and get achievements for both slots.

I believe you have to have the items in all slots at the same time.

no you dont. got the achievement with a itemlevel 200 belt on and a itemlevel 200 trinket on.

you are just needed to equip a ilvl 213 item in the slot and then u can switch back.
as a example the belt. my frostresist belt is ilvl 213. so that gave me that slot, and i got one trinket that is ilvl 213. so i just put that trinket in the first slot and the second slot so i got the 2 trinket slots filled:P

The maximum recover limit seems to be around 3-5 per account tho i may depend on how the item is being recovered?
