Are you using localhost or 127.0.0.1? Shouldn't make a different but 127.0.0.1 guarantees it's connecting locally.
How does your resources look? Call me paranoid, but did you accidentally download a virus?

And jesus, I hope not. Virus scan hasn't picked anything up, and I mentioned above that with the client and server running simultaneously (along with all my other normal processes like nvidia backend, windows stuff, etc.), I only utilize 4GB of my 16GB RAM and about 16% CPU. By all accounts there is no logical reason for this nonsense. It's incredibly frustrating and that's why I continue to pursue some kind of explanation. It simply shouldn't be.

I don't have a "world" window. I have realmd and mangosd.

No worries bro. I'm sorry I'm such a huge noob and you have to explain things like I'm five. What specifically am I looking for in that window, by the way? Every time I check it, it looks like stuff is just loading as needed.

Decided to try a new diagnostic tactic since I booted up the server and was able to maintain double-digit latency for a little while. I let the server run for about twenty minutes before logging in to a level 1 undead in the starting zone. 15ms latency for a few minutes. Ported to Undercity and only saw it jump to 17ms. Used the .levelup command, talked to some vendors and questgivers, visited the bank. Eventually it climbed to 56ms and stayed steady there.
I logged out, rolled a tauren, and decided to start questing to see how long it would stay stable. Pretty much not at all! Tossing a few lightning bolts at some plainstriders saw my latency climb back up to around a thousand. Even if I cease combat and interaction and just stand there, it continues to steadily rise. WTF.
Edit again: It's slowly dropping now, the longer I stay logged in?! Even if I'm doing stuff? I'm so confused.

what i can recommend im sure you may have tried but re-download the repack and just run it stock settings Could be that simple as i looked at both .config files tho i saw nothing wrong always a chance im wrong. when i ran test on my computer had 0-1ms running it stock settings. Hope we get this worked out for you soon

You do some activities that set processes in the emulator running and turning. Latency is an average over a period of time so probably spikes bringing up the latency and then reduces over time as more readings bring the average now. Sounds like something is reaching capacity when you do activities in game - check taskmgr and the cpu usage / RAM usage / harddrive usage when it starts lagging.
