could not connect database

hi,
I used this account creation page http://www.mmowned.com/forums/world-...tion-page.html

on my website yourperfectgame.com to create an account on my server but when filled in all the field it says : Connection to Database failed

This is the file:

can somebody tell me what i did wrong or what i have to do?

find a database which has a table called "accounts" or something similar and then use that Database name which the account's table is in
