Help with Vote page

Hi there, im trying to make a voting page for my WoW server, and i need some help. How do i make a vote page on my website (to make the question short). I have everything thats needed, MySQL, WoW server (arcemu) and a website.

3. Paste it into your website somewhere.

5. Profit!

lol. Anyways, Thats the best I can do. I cant tell you exactly where on your website as I dont know what website your using or what effect your looking for.
