[HELP] Account creation page public

Hey guys ^^
Can someone paste an Index.php in detail please?
like.. saying what everything is in exact detail :P
Ive looked at alot of guides and either theres something wrong with my computer or none of them are understandable :P

Another thing, for $server would i need to put what other people put in their realmlist or what my DnS is?
cause the realmlist is 78.146.59.199 and my DnS is Historicwow.servegame.org :S
really confusing for a first time public serverer :P

Everything works on my computer (i can get on the account page and make an account) but i tied on my other computer and it wouldnt work : /
