Voting Rewards

Well, after TONS of searching, and googling (literally months) I'm coming to the respective community of emulator specialists that I've come to know and love.
I'm looking for a voting popup that of which ani-wow, burningwow, and many others have. The one where you put in your account info, and get rewarded. I seriously have searched every emu site I can think of time, and time again. I'm not asking for anything too glitzy or glamerous, just code really, the rest I can manage myself. If someone could give me something basic I'd rep you forever.
Please everyone, I can really use this for my server, and I don't ask for things unless I find NO other way to perform it. If anyone can help me with this I would GREATLY appreciate it.

Yes, I'm looking for the one that will give players rewards. I have one for donations, but. I've looked over the script source code several times, VERY thoroughly, and for the life of me, can not seem to successfully convert it for voting instead of donations. If anyone can help me with this, i swear, i'll love you forever [In the most non-gay way possible] :P

yes, I've already scripted the pop up, and all of that, really, if someone could just give me the code, and explain to me how to implement it, i can do it from there.
