Submit a ban appeal
Your account was permanently terminated for violations of the Pokémon GO Terms of Service. This includes, but is not limited to: falsifying your location, using emulators, modified or unofficial software and/or accessing Pokémon GO clients or backends in an unauthorized manner including through the use of third party software.
Our goal is to provide a fair, fun and legitimate game experience for everyone. We will continue to work with all of you to improve the quality of the gameplay, including ongoing optimization and fine tuning of our anti-cheat system. If you believe your account has been terminated in error incorrectly, you can appeal that termination through this form. For privacy reasons, please do not post appeals on social media.

but those who cant log in. did not receive any email from niantic yet. so should not assume is a ban yet.
So far my banned accounts are PTC account. google accounts are fine for me.

"Failed to get game data from the server" on 1 gmail acct and 1 PTC acct for about 36 hours. No email received yet.

Please do not use that form, unless you have specifically gotten either:
- a message upon login, that you are banned
- have received an Email stating your account has been banned.
By sending a Ban Appeal, you are admitting you have cheated and want your account back. Noone who has not cheated, not received a banned notification would submit a "Ban Appeal". So why take the risk?
If at all, you want to ask for help, ask Niantic's normal customer service.

Do not appeal unless you have a msg from them saying you are banned.

I never click into that link to appeal my ban. Who care about it? Its just a little game. Just quit the game if both of my two accounts were banned. My second account can't login for whole from morning until now but I still got my main account there. Next month I will quit Pokemon GO even my main account is still there because I want to concentrate to my next game - Final Fantasy XV.

That message does not necessarily mean a ban. It could very well be related to server issues as they are a bit unstable at the moment.
