I did kill him hundreds time manually on first weeks of d3. Flawless gems were selling 5-10k so it was great spot to get some easy essence and gems

If you manually farmed sarkoth you will be safe. However, I think if ur character walked the same path again and again to get to sarkoth, chances are u will be flagged.
