A lot of people are having this problem, double check you installed the files right (This is a more advanced script than what youre used to)
There are PNGs at the bottom of the OP to help.

Its the Black text at the bottom of the screen. If you dont have that try using v1.2, it should create an actual log wherever you have the script at.

i using 64bit windows, i tried install and run the program in x86 but dont seem to work, i press "x" it can exit but dont seem to run when pressing "="
do i need to do a restart?

I just released 1.2, see if that helps.
If not Double check that everything is installed correctly. There is a set of PNGs at the bottom of the OP to help.

After 60 runs or so I'm still not picking up any gems.

Any advice on which/how to adjust tolerances? Its picking up Magic items just fine and having no other issues.

Use the INI file it generates... you should not edit the actual script at all.

There was an update just 10 minutes ago. You can try that. Also double check you are using the latest PNGs.
I know at one point the pngs were named incorrectly for some resolutions. Best to just get the new one
If those done help, the tolerances can be found near line 481

While _ImageSearchArea($pngLoc & "square.png",1,0,0,@DesktopWidth,@DesktopHeight,$gX,$gY,115)

Try making the 115 something higher. but do not go above 255. If it still doesnt work at 255 we have a different problem than tolerances.

I tried 1.2. My native resolution is 1680x1050. However I changed my resolution to 1600x1900 both in game and my desktop resolution. Right now when I press "=" the mouse is dragged to the bottom right corner and stays there.

You shouldnt have to change your resolution, if your mouse is moving to the bottom right corner that means that the PNGs arent being found still. Edit your INI to show where the pngs are. you can see an example INI at the top of the OP.

1.2 was released, it has a physical log as well as fixing some things for people with a smaller GF radius than I have.
Also fixed a few small bugs
Also most likely created a few more.

Is Diablo 3 running? I was getting something similar to that when it was not running and I ran the script.

You dont, its just there if you want to test adding your own custom items to the loot. You can drop them in town and pick them up rather than waiting for one to drop naturally. Things like potions, armors with a specific affix, etc.
