After saving changes in Noggit? What next?

So lets take for example situation. I move the fence near Gold Shire. I save and a .adt file appear to my map --> azeroth folder. What do I need to do next to get the change to game?
Also when I have got it in game, how I make a patch that allows other player(private server) to see it?
