These account retrieval forms are for those of you who have a banned account due to things such as sharing an account or having a scammed one which was banned because of a report from the original owner. People who have access to the email registered to the account will receive these documents when there accounts are banned due to account sharing and such. Mine were received when i tried to scam someone for some WoW time.

All you need to do is send in these forms with a copy of your picture id(or a fake one if your account is a scammed one) or a birth certificate or school id if u are under 18 and fax or mail it in, and eventually your account should become unbanned.

this is for if an account you scam or have and you do not have the email for this and the account becomes banned for something such as account sharing. you can use this along with an id to get the account back. I was just sharing it for those who don't get one, because this comes via email from blizzard to the account's registered email.
