No you dont need the same address and the SQ/SA is wrong i just remember i did a transfer to transfer account and i choose my own sq/sa and just had full name wasnt scammed btw it was legit and it worked maybe they changed it it was in march of 2008 this year

you can only transfer from 1 pvp realm to another.. you cannot go to an RP realm from a PvP realm

Only the names have to match up. Everything else is free game.
