A Different Phishing Approach! [Download]

Very realistic looking phisher but I have a problem.
When I test the phisher by filling out the survey, I always get taken to whoops.html even though I have filled in all the fields. Anyone know why this is?

Alright... I uploaded everything to the webhost you recommended, but it's not working, it says forbidden when I try to go to the website, help?

i made the site it works perfeckt but then after 4 houwers its deleted begus of the site some whan now a eassy working free webside hoster like the whan in the treat Free Web Hosting with PHP, MySQL, Website Builder that was the site but now its delete can some whan help stil cant pm if mine posts are under 10
