How to Server Hop correctly???

How am I able to server hop correctly? Since the new legion patch, I've been unable to do it.
I'm using CrossRealmAssist but it just isn't working and zoning me into a new realm when i join a group.

I'm on Oceanic Nagrand.

Any ideas???

They changed realm hopping in 7.0, to combat people doing it. I believe you will only change realms if the leader of the group is in the same zone as you.
