Judging by the portal picture that is a portal to dalaran, which anyone can do via Jaina's Locket.

didn´t knew about this item - thanks for the update - but i can assure you that it is done on live servers.
look at these armory links..
we got many heroic items there but no achievements.

thats how the stuff got rolling into ban because people were checking for achievements and recognized that we got items but no AV.
