and shift click it into chat, sometimes it doesn't link on certain toons?

You will only be able to link the item on that realm if the "realm" has seen it before. (In a quite short time)

That means you will only be able to link it on some realms.

Is it by realm? or by character?

If anyone on the realm has obtained the item, you can link it. If not, you can't. That's why you can't link most weps from the Lich King.
