It's not fixed, done it several times and works with any item. ONLY if the Transmogger is on a different realm than you.

And another requirement is that the item cannot be a class item that does not match with your class, i.e Warrior / Paladin Tier.

Sort of, they patched the transmog bug to be done on the same realm, however doing it while both are on different realms from each other, the transmog bug works just flawless.

Only requirement of course being is that the item you transmog must be useable by your class, so a Warrior cannot have a transmog of a Paladin Tier Piece.
