Can someone else confirm besides Spikeeen? Or can you post your rotation spike? As posted, the way I described in my post, this DOES NOT WORK. There must be a detail not mentioned. I tested it twice.

Have 3/3 DRM (BBFFUU)
Obliterate (BB-F-U)
wait 10s ... (BBDFDU)
Plague Strike (BBDFD-)
Plague Strike (BBDF--)

Respec to 0/0/0 (BBDFUU)
Use anything that will consume death rune (BB-FUU)
wait 10 seconds (BBFFUU)

Also, did you actually test to confirm this isn't an interface/client bug? I noticed in your screenshot you used an Icy Touch to test your death rune, but the behavior of death runes is that they are used only when the default type is not available. Therefore, when you casted Icy Touch your Frost rune should have gone on cooldown, hinting at the possibility that your death rune really is just a frost rune. What you should do is log on and simply attempt to cast three Plague Strikes in a row and screenshot that (along with combat log times). If you can do that, you can confirm with absolute certainty that you really do have a permanent death rune.

Personally I cannot make it work.
I tried this and specced for 3/3 DRM (53 pts in blood total)

at the start I had BB/FF/UU
went to a dummy, cast Obliterate on it
BB/F/U at this point
Waited till they refresh
BB/DF/DU
Plague Strike
BB/DF/D
Plague Strike
BB/DF/--
Wait till refresh
BB/DF/UU

at this point I respecced to another blood build with no points at all in DRM

BB/DF/UU still
Icy Touch
BB/D/UU
Icy Touch
BB/--/UU
Refresh
BB/FF/UU

I don't know if I did something wrong but it didin't seem like it worked for me.
