Unless there is something I am missing, you could of very easily faked these by attacking something till you got a point and ran there and took a SS. It would be a bit more believable if you had timestamps in your chat.

Give us a screenshot of the combat log, including the skill point in the log, and the target you attacked, and the attack used. THEN we can analyse what happened.

No, Blizz has stated you aren't supposed to be able to level off these. In fact, they even fixed most of the NPC mobs that you used to be able to take advantage of and level weapons.
