You can also just make a healer and heal your guy when you take damage. But, this works too

dnt know, because by the thing shes running on at begging looks like private serrver or could jut be model edit and we dnt know how far ur running out to sea that could of beed the first bit of fatigue dmg u took doesn't mean that u've been taking loads of damage

If you look at the second picture you see that she regenerates 7hp every second and she's lvl 1: fatigue does about 5-20 dmg (not really sure how much, been a while when I was exploring ) every 2 or so seconds.
So if fatigue damages you 10hp every 2 seconds, you get over half of the hp in 1 sec and rest before the next damage comes, so you can't die.
