For my druid, I got the Track Humanoids (Cat Form) early. I don't know how, but I had it 4 levels earlier then I should I have.

Had that one too, still haven't worked out how it happens, Maybe some peeps with spare time could bust this one open.

Maybe addon related i had Rico's Minimap addon at the time.

Do you use any if so what minimap mod you using ?
