Now, I'm no super magic wizard at C++ (though I'm working on actually becoming fluent with it), but your two requests, as GP said, are both possible. If you're the one who wrote a script for the item-adding thing, then surely you can do something like your second request. You just have to list it down in a logical process.
Item Adding: Add an item to every online player once an hour.
Player Debuff: Think of it like this.
Debuff Script.
- Revive a player, and apply a debuff.
- Does the player already have a debuff, or not?
- If not, then apply said debuff onto player, otherwise/else, delete hearthstone in their inventory
- If there is no hearthstone in their inventory, leave it at that
Sort of like that. Like I said, not a wizard, just logical thought process. Hope I could help a little bit.
Edit: This is assuming that you're the one who wrote the script.

Caeli Absolutio.
Picking back up C++, learning Lua later.
