[Request] User control panel

Can anyone make account control panel for tools. Like one scapegaming have.

I just need the login form that will use In-game accounts and take you to page where i can put player tools like: unstuck,transfer,ban checker...
And below input form to be like "Forgot your password?" for link to retrieve password script.
Dont need pictures just form to take you to next page.

Pls if anyone have time to make one or have link to download.
