There is a small chance that you're using an entry ID that has been used on another server you play on... just delete your cache folder and log in again If it's still like that... just delete the item and recreate it.

your cache is in your wow folder......its a folder named "cache"...just delete that folder and log on

u most likey put the MH slot for the head slot thats why it only goes in your mh make sure u look at that i do it all the time
