I'll try to take this list seriously, and note down a few points I find to be important.

For applicants under 16, they will have to provide a scan of their report card. (As suggested by a person above. Smart idea)
All applicants should have proper grammar. Forexample, in a text field, they should be able to see the difference between these two sentences.
All students who have delivered their report could leave.
All students, who have delivered their report, could leave.
And additionally, know the difference between when to use are and when to use is.

The reason I find grammar/language to be so important is because communication is important. And people not writing properly puts the entire server in a bad light.

When to shut up, and when to argue. I've met GMs who called me a racist for saying something was nazi. He even started to argue with me, and it resulted in me being banned.
While I understand that GMs find arguing players to be important, it's just stupid to be wrong and keep arguing that you are right, especially when evidence has been delivered.

I honestly cannot say I've got a lot of questions for your test. In general, they should have ones taking up situations and writing down what's the correct thing to do. How to do what, when to do that, etc.

Since I'm writing this while being hungry and tired, please excuse me for any flaws of logic, grammar or anything like that.

    Good luck!

The way they see it, if there doing all that work shouldn't they have fun? More fun then the rest of the people? Plus whats the worst thing that can happen? Get your powers taken away? Get banned? Find another server and rinse and repeat.

On internetz your age is unknown, so you are mostly judged on your mental age, so mental age is more important imo. But since I'm immortal and over 2000 I cba to care about age and stuff.

I liked the idea of having younger applicants showing you their semester grades. In the case of an older member, what are you going to request? His Bank statements?
A Gm should be selected based upon quality of service and NOT quantity. It should also be based upon how well he/she handles stress. If they suck at understanding when they are wrong, or buckling under a particularly heavy weekend, they are not suitable.
But look out for the ones that are particularly GOOD under stress, they wont handle the quiet nights very well. So basically look for a well rounded, mature hardworking individual who takes their time with tickets and other responsibilities instead of rushing, or doing it for the wrong reason such as an item reward, It should be done because they like helping people and no other reason. They are voluntary after all. Volunteers don't get paid.

In my opinion you really shouldn't look to age at all. I mean, I've had my own server and I've been (senior)GM on a few servers now, and as far as I know I've never used any power for advantages or whatsoever, neither did I get any real complains. And yes, I'm 15.

Sure, you can look to age, but try to get your opinion based on the other things like grammar, experience, school (as in, if it's a high education they're most likely more mature) and how the act ingame towards others.

I work with several GMs and even Managers under 17 on my server, some of which were may more mature and responsible than plenty of 20+ or even 30+ players. Age doesn't mean much, maturity and responsibility do.
Looking at how someone behaves in-game or on your server's forums is usually the best way to get a good idea of how someone behaves. Perhaps go undercover on a player toon once in a while so you can see how people act when there is no GM around.

What you want to ask them in your applications up to you, though honestly I don't think there's much point in asking them 125 questions. Just ask some of the common ones (Why are you applying for this position? What would make you a good GM? What have you already contributed to the server and what more will you be doing when you are a GM? Describe what you think are the most important tasks as a GM. What are your main flaws and how would they impact on your role as a GM?)

One thing I'd like to add, many servers ask applicants about their knowledge of commands or even require applicants to be familiar with them. To be honest, I never saw the point in that. GM commands are simple, the basics can be taught in a few hours and they'll learn the more advanced stuff when they are GMing in-game. Unless you are looking for specific knowledge (SQL, lua, C++, whatever) just don't bother asking about their knowledge at all.

Age doesn't matter, as the player can be a 14 years old kid and act as an adult. He could put in his application that he's a 25 year old unemployed rich guy that play WoW all day long, and is respectful, polite, blah, blah, blah.People would believe, and there's a high chance of him getting the GM position, as most server doesn't requires the player to give a photo of himself.

Also, I GM'ed a WoW server when I was 15. It was on the Top 5, and I told them I was 20. They believed and hired me, and I did the job accordingly. But, because they were lacking Gms, they hired like 20 at once. About 10 got kicked in the very first day, and 5 in the first week. Me and the other 4 guys remained in there until the server Main GM went nerdrage-ish, argued with the admin and the other Gms, banned everyone, including the admin (Which was a Cry-baby), me and all the players. That's the only reason I stopped GMing in there as a kid.

personally ive been a Gm,Admin on a few servers and ive never taken advantage of powers.
But i do agree that with age comes responsibility, the older you become the wiser and more responsible you should be. In some cases you can get a 21 year old that acts like he 10.
Just because a 13 year old can drive a car would you want him to be out on the road ?
