Need help with site account creation.

hey and thank you for reading this.

me and a couple of my friends got the server going did all the changes and all the stuff we need done on there now we are to the website. we have paid a company to host it each month for us but we get to customize and all of that. we go to set up account creation and it just doesnt wanna work for us r scrippting must be wrong somewhere. can someone give me a proper script for this problem say that when we upload it on the site it will work properly and not half ass like it does now. i would very much appreciate it.
btw server is a acent. if that means anything for the script.
