[PQR] Demonology Tank Profile

Hello. I'm looking (as in tittle) for complete demonology tanking profile - Dark Apotheosis form from [Glyph of Demon Hunting]. I was currently working on my own profile but there are some issues I cant get to working state. My character cant use properly Demonic Slash skill. I have no clue about this, but i think there is something with spell id. Same is with Fury Ward. Please help me out with this guys.
