in the thread titled "First known Perma ban" it's assumed whoever created the image simply got the "banned" image from the game files as there is no evidence to back up the claim.

I personally really hope I won't be included in a mass ban if it happens, I use my phone for real and also spoof at home, I have never jumped to a location outside of the areas I have actually been with my real phone never left anywhere outside of my city and nearby locations to my city. Never jumped more than even half a km and have not be soft banned at all.
I have spoofed in the past on ingress many times with the same ruleset that I'm following on this game and have never been banned... so I really don't know if it's possible they'll be able to catch you if you follow a strict ruleset such as I.
I too am worried about a mass ban.
Beside detecting that a player violated the laws of physics by traveling too fast to a different location, Niantic could potentially identify cheaters in these ways:
- Traveling in impossible / difficult areas (going across water, crossing where no paths exist, entering buildings where there are no doors or when they are closed, etc).
- Always traveling at the Nox speed
- Accessing areas such as Central Park that are known for the ammount & quality of their pokemons.
- GPS too precise or connection too reliable, especially while inside indoor areas
- Battery level constant / charging while on the go
- Mouse detected instead of touch screen
- GPS spoofing apps getting detected
- Nox / Bluestacks getting detected (Avoid possible Ban for NOX by Change your Phone Type / Resoultion)
Niantic could easily identify accounts that are suspect (for example accounts that got soft banned & accessed high Pokemon areas) & run more exhaustive algorithms on these accounts to confirm cheaters.
On the plus side, Niantic's servers have been overwhelmed since Pokemon Go was released. Hopefully they disabled their more sophisticated cheat detection algorithms to reduce the workload of the servers.

Are the permabans confirmed, I feel like there is no legitimate source to prove this. People spoofed in ingress without getting banned, so I feel like this might just be photoshoped

yeah there's basically no proof to back up any claims being made

As long as you:
Haven't been softbanned
Haven't spoofed far outside of your town/region
Waited between jumps
You're more than likely fine.

I highly doubt it, but it's better safe than sorry. I wouldn't be surprised if they marked current soft banned accounts for further investigation. But then again, that would be a SHITLOAD of people. We will find out soon enough.
