You receive donation rank for the duration you donated for, after that you revert back to your previous rank.

Yes, in order to get your donator rank and access to those certain parts of the forums you will have to donate again.

If you look at http://www.ownedcore.com/forums/payments.php you see that you can choose between 3,6 and 12 months to become a donator.
