[Request] Stream Chat Blocker/WoW Informational Banner

Alrighty, so I've just recently started streaming and I'd like a small banner with my stream name on it and my character showing aswell.

One of my IRL friends uses one like this: Clickmeheheprohtmljkauto.
And I'd like one looking very much alike it but of course the colors can be different and without the "Gladiator Icons" above the name.

Stream name: "Mayxlol"
Colors: "Dark Blue/Cyan/blue.. so just blue ish colors I guess"

Extra text: Would be nice if you could put "Make sure to "Follow" the stream" or something like that under the name
