[Question/Request] Trinket Graphic Change

I'm looking to change the graphic for the Hex Shrunken Head (also the Icon of the Silver Crescent) when you activate it to that of the Natural Alignment Crystal.

I found a guide on how to do this, however, I can't find the spell name for either item's effect.

So my request would be someone do this for me, but I'm not so lazy as to just ask for it. If someone could just point me to the correct spell name for both, I can do it myself.
