Demon Form IDs

So i'm looking for the Display ID and the Model ID (or are they the same?) of the warlock demon form, yes the purple one, for some NPCs and model swaps and such. Rep will be rewarded in turn for the IDs.

This would be very good to know. I tried to do a simple spell swap (using one of the tools available on these forums) from fel armor to Metamorphosis, but it didn't change the model, just the particle effects (cloud of smoke and some purple lightning) immediately after cast.

Playing in demon form all the time would be very nice, but it is beyond my (limited) knowledge.
