In-game texture problem with a Custom Model

I've been working on this custom model of Gabranth's Helmet from Final Fantasy XII. The texture on the model shows up just fine in the model viewer and in 3dsmax, but unfortunately, the texture is horribly malformed in game.

Screenshot of the MPQ loaded in wowmodelviewer:

    Screenshot in-game:

There have been a couple other models of mine this has been happening on, and I was wondering if anyone knew what was happening, or how to fix it.

You may provide the .m2. People can help better if they have more than a picture.

    Here's the MPQ: patch-H.MPQ

Replaces the Tier 8 Blood Elf Male Death Knight Helm.

Thing is: The texture on the second screenshot isn't even the one in the MPQ, showing that WoW seems to load an other texture than yours. Try to swap the helmet with another one. If the "texture" will be something else, it seems like its loading the texture from ItemDisplayInfo.dbc even if its hardcoded. I don't know why it should do that, but meeh.

I was thinking that WoW was using a different texture than the one that I was applying to it, so I ran texmod(a program used to rip textures from games) and tried running it on WoW. This is the result:

The green part represents the texture that I have selected, and the texture in the top left corner is the texture that's being applied to the green part. I believe this means that it's getting the right texture, but it's being horribly skewed somehow.

Also, using the EXACT SAME MPQ file, the helm looks like this in game now for some reason:
