Sorry in advance if this is a repost or so.

How to get it? That's easy peasy. All you need is some sort of shrinking tool (toy, potion etc). I used Gnomeregan Pride.

Smallest size can be achieved with Whole-Body Shrinka'. Smaller than critter.

Im not sure how this is so sneaky... when the guards go on break, the ENTIRE room is flooded with enemy nameplates. I use default UI with no addons so even if YOU are tiny, your nameplate will be default size, easy to click, and a piece of cake you yank you out from under there.

Wish the pic was a tad more descriptive. Can I loot from under that deck or do I need 2 jump on stuff? ALso when does this chest spawn

Sorry for the pic, I'll try to explain.
The spot I've used is behind the piece of cloth/leather or whatever it is. Simply run behind it, use a shrinking method that suits you and you can loot the chest from underneath.
You're immune to AoE and LoS from 99.9% of everything. Only way you'd get attacked is if someone runs in where you stand, which I've only found one entrance to do so (depending on character model size).
The chest spawns after/during rares. Most people will focus on AoE to keep everyone off the chest.
