Base Set for WotLK:
Head: Headpiece of the Shattered Vale
Neck: Necklace of the Deep
Shoulder: Spaulders of the Shattered Vale
Back: Fen-Yu, Fury of Xuen
Chest: Bloodsea Brigand's Vest
Wrist: Armbands of Dark Determination
Hands: Gloves of the Endless Dark
Waist: Soul-Devouring Cinch
Legs: Leggings of the Immortal Night
Feet: Footpads of Impending Death
Finger 1: Spellbound Runic Band of Unrelenting Slaughter
Finger 2: Ashen Band of Endless Vengeance
Trinket 1: Mirror of the Blademaster
Trinket 2: Ghost Iron Dragonling
Two-Hand: Gao-Rei, Staff of the Legendary Protector

For TBC, use the base set with these replacements:
Legs: Fel Leather Leggings
Feet: Fel Leather Boots

For Cataclysm, use the base set with these replacements:
Legs: Bladeborn Leggings
Finger 2: Loop of the Agile

Gems & Enchants:
Stack crit up to 52% (crit cap), then stack agi.

Get multistrike from the WoD enchants, and look at the tooltips/links for the other

How did you determine the best gear with simcraft?

I found a list from FluidDruid forums as a base, then compiled a list of every item with 2+ gem slots, then ran an automation script in SimCraft with all the set variations and found the top scoring one.
