This is caused by the paladin either bugging his vengence so it has over a million AP then being teleported to the location of the mob meaning 1-shot kill. The bug happens in some 85 raids.

pretty has been hotfixed was some issues, basically a pally on say herioc madness where the impale would normally gib you without cds giving massive amounts of vengence, (as vengence now gives ap based on the incoming dmg regards of dmg reduced via armour/dodge/cds etc)
