When you use Orb of the Sin'dorei you become a male or female depending on your gender, but if you have a costume that turns you specifically into a male, or female, and you use the orb, you will become a blood elf of that gender. I had the Leper Gnome costume on on my female character and when I used the orb I became a male blood elf. Ta-daa.
