Hey, stumbled a crossed this thread. If the OP or anyone else that might know this issue, let me know. it seems very close to the same issue the OP posted.
anyways, I am the author of this program EverMorph. not sure if you follow the project at all but along with spawning in-game objects I have been working with a feature to unload and load maps at a wim.
Most maps I can load with some pretty disgusting code but it seems a few (the development ones) give me that exact same issue that the OP posted. I still have some things I might try such as not rendering any models and a few other tricks.
I would really like to make this a public release, if anyone knows what causes said issue please open thy mouth

The crashes are no longer, and were easily fixed once figured out, it was so obvious that it remained mysterious/undetected for a while:
It was a simple file naming error (due to changes to how ADTs are numbered in Cataclysm)

in case you didn't see this, it's what im working on as another evermorph feature..
it loads 80% of the maps fine, but the issue I have is almost identical to the one you posted. If i am not running under a debugger, its a disconnect back to login screen (as you said) and this is as of 4.2.
Im a little new to reversing this game but I have made a dump of Map database client, and the ones that generally fail to load are map 451 (development) CraigsTest, and a few others.

Correct. (not sure about some PTR revisions, they are rumored to have had some of these maps)Development maps are not in the shipped client by default, iirc.
To those who can't wait: As you already know, those files are special to my/others eyes, development being the most anticipated and desired but also the hardest map to get/explore ever, source of many hopes and disappointments. Holding the files back may sound cruel to your/others ears, but be aware that I could also have left them private forever, seeing I'm the only one (among the very few others who own them) willing to ever release them to public.

Map.dbc still references some old/unused/broken/deleted maps entries which will fail to load using the retail client due to missing data.
With WoW 3.X & older, any missing/corrupted data will shutdown the client silently/unhandled_exception #132.
With WoW 4.X & newer, any missing/corrupted data will send back to login screen without killing WoW process/unhandled_exception #132.
They should be pretty obvious (the tests/devs/transports ones) but I can make a list of IDs to avoid if you like.

The chances are not very high, if not very close to zero.
The first and only time they were available (4.0 ptr if I remember well), they were deleted 1 hour after.
So, if Bli² ever decides to put them again (which would be kinda silly), you wouldn't have much time to save them.
