It would be more ideal to preset it to a hotkey and just click the hotkey right when you die, if you have the mod change depending on your form you might already be in combat again.

Alls you really have to do is put the card in any of the hotboxes... you have the entire time in spirit form to click it and switch over, just make sure the trinket you want it to replace for that time is your upper trinket.
