The exploit here is, that you can heal Everyone, even duelers!
If your a shaman, and you play on a private server, you buy Tier 1, and then you get the itemset bonus:

Chain Healing Wave - Spells - World of Warcraft

if you got the items, then you have to use Healing Wave rank 1-10, else it wont work. Check out this picture.
