help with my site creating a sumit form

ok so here is what I have The Fallen Gods Home its the little submit form at the bottom of the page

ok so what I need to do is have it work now does anyone who a way I can set this up to work the way it look when you try to use it now it wants to send it as a email but there is no message if everything that was submitted sent in the email or something that would be cool but its only working as a email link can someone tell me how to set it up into the sql or something

so can someone help me script this to work how ever possible & ill +rep you what ever I can
